Wisconsin health officials are making COVID-19 vaccine shots available to anyone age 65 and older starting on Monday, Jan. 25.

Here's how Washington and Ozaukee County residents 65 and older can register for an email distribution list to learn more about getting the vaccine.

  • Go to the public health department webpage at washozwi.gov
  • Below the "Mission" and "Vision" banners you will see the "Coronavirus (COVID-19) Information and Updates" banner.
  • Where it says "If you are 65 years of age or older, please complete this form," click on the hyperlink included in the word "form."
  • That link will take you to another webpage. You can also click on this link to find the form.

Once on the form webpage, which is titled "Vaccination Distribution Contact List," you can fill out your name, email, phone number, age group, employer name and job title.

After you fill in that information click on "submit."

It's important to note that submitting the form won't immediately register you for an appointment to get the vaccine. Rather, every Monday, when the health department finds out how many doses of the vaccine they received from the state, they'll send another email to some residents so they can then register for an actual appointment.

If residents ages 65 and older don't have internet access, they can call the health department and request to get signed up for alerts. The phone number for the Port Washington office is 262-284-8170, while the West Bend office phone number is 262-335-4462.

Kirsten Johnson, the director of the Washington Ozaukee Public Health Department, said the department is working on updating their registration system. They're also telling residents to check their website first if they have any questions about the vaccine rollout.
